Based on utilization review models used by hospitals to monitor DRG-related care, Service Utilization Review (S.U.R.) offers the same level of clinical and fiscal control to the home health provider.
OBQI CASE CONFERENCE - A uniquely structured version of the mechanism used to disperse clinical information throughout the home health team while prompting both quality care delivery and successful outcomes. This program was developed through years of refinement of the standard case conference routine to address PPS and other Medicare home health reforms. It will facilitate OBQI score improvement, staff and patient satisfaction and fiscal results instantly upon implementation.
PPS NEW RULE EDUCATION - HHSM offers the definitive New Rule preparation program for agencies who seek to manage the reform to achieve fiscal and clinical efficiency. - 1 Day Educational seminar - Introduces HHSM product information includes 2008 New Rule changes, how to focus on the OASIS tool for service utilization review and OBQI case conferences, HHRG training (what you really need to know NOW), system management.
- 2 Day Educational seminar instruction - In depth and intensive instruction and development of HHSM products (S.U.R., OBQI Case Conference, New Rule Strategies, Therapy Utilization) as used in multiple agencies and described in both national publications and educational venues. Allows self-motivated agencies to adopt HHSM techniques for success and achieve elite agency status
- On-Site Consulting Level I - In 15 to 30 days modulated on-site by HHSM staff 2 days/ week with fiscal outcomes of case conferences provided and S.U.R. instruction to your staff for on-going performance. Case conferences to be scheduled by the agency personnel and run by HHSM staff once before turning the program over to staff to continue.
- On-Site Consulting Level II / Coaching - In 30 to 60 days, HHSM gives hands-on expertise in running the case conference model with on-site visits 2 -3 days/ week initially as case conferences begin. Gradiated lessening of HHSM staff involvement in weekly case conferences allows a flattening of the learning curve so that the program is taken over internally with in a month. Support continues so that customized needs are met.
- Management Intervention Service - 60 to 90 days with a 1 month and 2 quarterly follow-up visits post-engagement. A true agency makeover, if you will. This will ensure hardwiring the all of the above services in your agency permanently with instruction, education and training of executive personnel, middle management and clinical staff to assure ongoing implementation of the HHSM program.
