- 1 – 3 Webinars – 60 mins each – address PDGM for Administrative, Management, and direct care staff w Q & A for each session.
- A PDGM UR analysis of 5 current cases – Identify areas of current care trends that must be addressed for PDGM success, to determine:
- OASIS Accuracy
- Plan of Care Integrity
- Safety-Based Frequencies
- Volume-based Programs
- Discharge for Outcomes level re Star Ratings/ HHCAPS/ Readmissions
- Development of PDGM Care Plans (POCs) – HHSM will work with the HH Provider at the onset of the PDGM era in term of the real-time development of ten (10) UR-based PDGM Care plans.

- PDGM POC – 30 day vs. 60 day

Delivered by HHSM President Arnie Cisneros, or Executive Clinical Director Kimberly McCormick, PDGM Prep Package #2 uses a remote model to help your agency prepare for PDGM. This includes:
- 1 – 3 Webinars – 60 mins each – address PDGM for Administrative, Management, and direct care staff w Q & A for each session.
- A PDGM UR analysis of 8 current cases – Identify areas of current care trends that must be addressed for PDGM success, to determine:
- OASIS Accuracy
- Plan of Care Integrity
- Safety-Based Frequencies
- Volume-based Programs
- Discharge for Outcomes level re Star Ratings/ HHCAPS/ Readmissions
- Onsite Education – HHSM President Arnie Cisneros or Executive Clinical Director Kimberly McCormick will spend 2 days onsite at your agency working directly with administrative, supervisory, and clinical staff to move PDGM programming forward.
